Fill out necessary patient information, previous medical history, and sign required forms. These forms are found under the questionnaire tab. 4. You can view popular services here. Does PHH take insurance? No, we do not take insurance. However, we encourage patients to take their receipt and file with their insurance. Sometimes insurance will cover the visit, but please be prepared to invest in your health with card or cash. Who will be my provider? Craig, the Physician Assistant is the main provider who oversees all appointments. Chandler is a Paramedic who will also be assisting with appointments and labs. How do I schedule an appointment? Please reach out to us via phone, or schedule an appointment online through our online booking software. Do you provide prescriptions for controlled substances? No, we do not prescribe controlled substances under any circumstances.
